NON IMMIGRANT VISAS

NIV Forms

All applicants for a nonimmigrant visa (NIV), including children, must present a separate, completed Form DS-156 at the time of the interview. Applicants, with the exception of minors (16 or younger,) the elderly, or the handicapped, are reminded to come unaccompanied to the Embassy. Due to limited space, only visa applicants will be allowed into the Embassy, all other family members, friends or colleagues will not be permitted entry.

All male nonimmigrant visa applicants between the ages of 16 and 45 must present a completed Form DS-157 in addition to Form DS-156.

All students applying for F1, M1 and J1 visas must present a completed Form DS-158 in addition to forms DS-156 and DS-157 (if applicable.) Application forms must be completed in English or French and must be signed by each applicant (parents may sign for children under fourteen years old).

If someone other than the applicant completes Form DS-156, the person completing the form must sign his/her name at the bottom of item #40 on Form DS-156.  The applicant must also sign the form, and is responsible for the accuracy of the information provided in the application forms.

NOTE: Incomplete applications will not be accepted. Visa section staff is prohibited from assisting with the completion of visa forms.

All applicants must complete the DS-156 application for nonimmigrant (temporary stay) visas online at https://evisaforms.state.gov/ds156.asp

Important: After completing the form, you must press the "CONTINUE" button at the bottom of the form, then print the form and bring all THREE PAGES to the U.S. Embassy on the day of your interview.

THERE MAY BE SERIOUS CONSEQUENCES IF YOU OR THE PERSON COMPLETING YOUR APPLICATION MISREPRESENTS YOU OR YOUR INTENTIONS ON YOUR APPLICATION FORM. IT MAY NOT ONLY ADVERSELY AFFECT YOUR CHANCES FOR OBTAINING A VISA, BUT COULD LEAD TO A PERMANENT INELIGIBILITY.
